[Academic] E-risk Assessment 4603 - Return to campus for teaching

Hi

I have added you to the e-risk assessment for on campus class teaching as you have been identified as someone who will be on campus teaching tutorials (some lectures) in Semester 1.

In order for this to be submitted on time to the Dean for approval I require  you to sign this document no later than 10 am tomorrow morning.

If you do not sign you will not have access to teach next week and will have to be resubmitted when you have done so.
