[Academic] Class Information Sheets

Hi Everyone

Can you please forward me a copy of the class information sheet you supplied to your students at the start of term for each course you are teaching.

It would also be helpful if you could provide me with the updated class information sheets for your Semester 2 classes.  If these have not been completed yet, then I would be obliged if you could please do so at your earliest convenience and no later than 4 October 2013.

I attach a blank form should you need to use this.
